Spanning the years 1900 to 1977, The Covenant of Water is set in Kerala, on South India’s Malabar Coast, and follows three generations of a family that suffers a peculiar affliction: in every generation, at least one person dies by drowning—and in Kerala, water is everywhere. At the turn of the century, a twelve-year-old girl from Kerala’s long-existing Christian community, grieving the death of her father, is sent by boat to her wedding, where she will meet her forty-year-old husband for the first time.

According to Confucius, “an educated woman is a worthless woman,” but Tan Yunxian—born into an elite family, yet haunted by death, separations, and loneliness—is being raised by her grandparents to be of use. Her grandmother is one of only a handful of female doctors in China, and she teaches Yunxian the pillars of Chinese medicine, the Four Examinations—looking, listening, touching, and asking—something a man can never do with a female patient.

In the spring of 2020, Lara’s three daughters return to the family's orchard in Northern Michigan. While picking cherries, they beg their mother to tell them the story of Peter Duke, a famous actor with whom she shared both a stage and a romance years before at a theater company called Tom Lake. In 1950, a young doctor called Norton Perina signs on with the anthropologist Paul Tallent for an expedition to the remote Micronesian island of Ivu'ivu in search of a rumored lost tribe. They succeed, finding not only that tribe but also a group of forest dwellers they dub "The Dreamers", who turn out to be fantastically long-lived but progressively more senile. Perina suspects the source of their longevity is a hard-to-find turtle; unable to resist the possibility of eternal life, he kills one and smuggles some meat back to the States.
